Required Documents

To change the name on your drivers license, you must present proof of identity as well as legal proof of your name change. The following documents must be presented to the DSD office:

  • Proof of identity (primary)
  • Proof of identity (secondary)
  • Birth certificate (original)
  • Legal documents proving name change (Marriage Certificate, Divorce Decree, Court Order, Certificates of Adoption, etc.)

Normally, the fee for a first-time change of name is $8. Subsequent changes cost $12. The fees may vary depending on which DSD office you go to. A driver can pay the fee via:

  • Cash
  • Check
  • Money order
  • Credit card (Visa, MasterCard, or Discover credit card).

Out-of-State Change of Name

If license holders are applying for a name change on their Tennessee drivers license while out of state, they will need to do so by mail. To apply by mail, you must be:

  • A military officer with proof of order that they are assigned out of state
  • Have a valid military ID
  • A missionary who is assigned out of state
