Vital Records

When applying for a Tennessee driver's license, one of the primary documents you can use to confirm your identity and legal status is a birth certificate. Birth certificates are available from the Tennessee Department of Health's Office of Vital Records. The office retains short form birth certificates as far back as 1949. Birth certificates are not public records and can only be accessed by the person named on the record, their immediate family or an authorized representative. If you need a copy of your birth certificate, you can get one in-person at the Office of Vital Records in Nashville, by mail or online. If you need your certificate in a hurry, you can pay a fee to expedite the request.
